Form for CCOs Annual Reporting of Abuse and Harrasment (A/H) Incidents towards Healthcare Workers

Please note that the form below is only for CCOs to submit a cumulative list of all AH incidents towards healthcare workers that occurred in the reporting calendar year (1 January – 31 December) to AIC/MOH.

Description

Please use this form only for submission of all AH incidents towards Healthcare Workers in the reporting calendar year (1 January – 31 December). Please note that all FormSG submissions should be sent in by 7 January.

 

For submission, please ensure that:

 

  • All required fields are completed in the annual batch reporting template.
  • The necessary approvals from your organisation’s CEO (or equivalent) and other appropriate approving authorities (if any) have been sought before submission to AIC/MOH.
  • The excel file is password-protected before being uploaded onto FormSG for submission, and password to be sent to AH Incidents (AIC).
