Streamline documentation process

The lack of user-friendly and streamlined modules within systems results in a highly time-consuming documentation process undertaken by Care staff.

 

Summary of observation findings:

 

  • Duplication of efforts as handover notes are a summarised version of notes keyed into system
  • Deciphering notes may be difficult and time-consuming if handwriting is illegible
  • Documentation fields are in separate forms, which require staff to navigate or open multiple tabs to complete documentation for one senior

 

*Findings were based on data from 3 CCOs covered during the work trial

Using templates for documentation, digitising handover notes, and using mobile-enabled applications for data entry can reduce inefficiencies in previous processes.

 

The desired outcomes include:

 

  • Reduced overall time taken for documentation
  • Freed up capacity for staff to take on value-adding tasks

Estimated 50% reduction of total documentation time from the:

 

  • Digitising of progress notes
  • Digitising of handover notes
  • Streamlining of vital signs, bowel movement, intake/outtake documentation
