The role of Community Care Track in the Care Ecosystem

The redesigned Community Care roles, equipped with eldercare knowledge from multiple disciplines, collaborate daily with other colleagues on-site, as part of a multi-disciplinary team.

Nursing Team

 

SCCAs work closely with the Nurses to ensure that clinical tasks, such as the serving of medicine and wound care, are completed in accordance to standards set by the organisation. The CCE and CCM may also raise concerns regarding the deterioration of the physical, cognitive, behavioural status of elders to the nurses.

 

 

Therapy Team

 

The Care Team may raise concerns regarding the deterioration of physical and mobility status of elders to their Therapy-trained colleagues. They may also consult with the Therapists to better understand the capabilities and limitations of elders in performing maintenance rehabilitation exercises.

 

 

Medical Social Workers

 

Especially in the Nursing Home setting, CCAs and SCCAs may work with the MSW, providing key information of their elders’ day-to-day lifestyle habits and general condition. This information would help the MSW to provide better tailored advise to Next-of-Kins (NOK). The CCE and CCM will also follow up on issues raised by the NOK relevant to the Care Team.

 

HQ Staff

 

HQ staff such as doctors, dietician and Allied Health Partners form a multi-disciplinary team that gathers to do a 6-monthly review of the elders’ Individual Care Plans. The Care Team supports them by providing inputs on elders’ condition observed in their day-to-day operations.
