Meet Our Community Care Professionals

Tang Li Wen

Senior Occupational Therapist
SPD

Li Wen knew early on that she wanted a career helping others. Today, as a Senior Occupational Therapist at SPD, a disability-focused charity, she conducts therapy sessions, mentors junior colleagues, and helps clients live as independently as possible.

She chose Community Care to support seniors over time, helping them return to what matters most. For her, the greater challenge is ensuring every senior feels genuinely seen and cared for, even beyond therapy sessions.

Li Wen also serves as a bridge watchkeeper on naval vessels with the SAF Volunteer Corps and was selected to march in Singapore's 60th National Day Parade. Both roles reflect the same belief: showing up, doing your part, and caring for others and the nation.

Aishah Yunus

Case Management Assistant Manager
Thye Hua Kwan (THK) Moral Charities

Aishah Yunus

Aishah is a social worker at THK Home Care Services. She supports seniors living at home who face challenges beyond physical health, from loneliness and caregiver stress to financial strain, family relationships and housing concerns.  Her role is to understand their needs and connect them with the right support at the right time. For seniors who cannot speak, she communicates through expressions, gestures, pictures and simple questions, always addressing them directly. To Aishah, everyone deserves to feel seen, valued and supported, especially through life's challenges.

Kenneth Ng

Centre Manager, CareElderly @ Circuit
Care Community Service Society

For Kenneth, Community Care is about making sure no senior feels forgotten or left behind. Through outreach efforts in the Circuit and Balam estates, he supports seniors from lower-income households, encouraging them to stay active and socially connected. By conversing in local dialects, he builds meaningful connections with seniors, helping them feel understood and valued.

Beyond regular visits, Kenneth takes time to listen and respond to seniors’ needs, making a meaningful difference in their lives. When a previously active senior withdrew from the community after a fall left her using a wheelchair, Kenneth helped her reconnect through regular pet therapy visits. Over time, she regained the confidence to venture outdoors and rejoin community activities.

For Kenneth, moments like these show how patience, care and encouragement can restore a senior’s confidence and sense of belonging.

R. Subramaniam

Senior Staff Nurse
Methodist Welfare Services

R. Subramaniam

Subra is a Senior Staff Nurse at Methodist Welfare Services. He carries two jobs at once: Home Care Nurse for seniors managing chronic conditions in the community, and Home Hospice Nurse for those in the final stages of life.

One visit may involve helping a senior regain strength and independence. Another may mean bringing comfort so they can share precious moments with family. For seniors who cannot speak, he reads body language, breathing patterns and subtle muscle movements, offering reassurance through touch and tone.

Some seniors initially refused his care, only to eventually wait by the door for his arrival. For Subra, earning that trust is what makes his work meaningful.

Lynette Lim

Senior Manager
Agency for Integrated Care

Lynette

For Lynette, reaching out to seniors begins with a friendly greeting and an interest in their lives. At the Silver Generation Office (Pasir Ris-Punggol), she supports outreach efforts, coordinates volunteers and connects seniors with schemes and services that may benefit them.

Beyond sharing information, she also takes time to listen to their stories, learn about their passions and understand their needs. These conversations help her build meaningful connections and make seniors feel heard and supported.

She recalls a senior who was initially sceptical about receiving help from community agencies. Through patience and repeated visits, she gradually earned his trust and willingness to accept support. For Lynette, moments like these show how a simple conversation can open the door to meaningful change.

Koh Yun Shi

Care Consultant
Agency for Integrated Care

Koh Yun Shi

Yun Shi, a Care Consultant at AIC Link @ Ng Teng Fong General Hospital, believes that quality care begins with understanding each person's unique circumstances. She helps seniors and their caregivers navigate care options and access the support they need to continue living well in the community.

Many of those she supports face health challenges, uncertainty and emotional stress. For Yun Shi, her role goes beyond connecting them with services. It is about listening to their concerns, understanding their needs and helping families feel reassured and better equipped to cope.

Through patience, regular follow-ups and genuine care, she helped a hesitant senior gradually open up and accept support. For her, helping someone feel heard and supported is what makes her work truly meaningful.

Gwen Neo

Music Therapist
Ren Ci Hospital

Gwen

Music is more than a source of comfort for Gwen. It’s a way to help seniors express themselves, connect with others and find strength through difficult times. Her role entails tailoring sessions to each senior’s emotional and physical needs, using familiar songs and musical activities to support their well-being.

Her work goes beyond what words can express. For seniors who are unable to speak, even the smallest response, from a smile to a flicker of movement, can signal a moment of connection.

During one session, a familiar song brought back memories for a senior and her daughter, allowing them to reminisce about happier times together. For Gwen, moments like these show how music can bring people closer, restore a sense of identity and offer comfort, even when their circumstances cannot be changed.
