Introduction 

 

Home Palliative Care supports patients with serious illnesses, like advanced dementia or cancer, with palliative care at home. It also supports their caregivers who may find it hard to manage their loved one’s changing needs during this period.

 

Who is this service for

 

This is for clients who:

  • Are homebound due to a serious illnesses diagnosed by a doctor
  • Have been diagnosed with a serious illness with a prognosis of less than a year

 

Services provided

 

Services may include:

  • Palliative care: Medical and nursing support to manage pain and symptoms
  • Psychosocial support: Emotional and social support for both patients and caregivers during this difficult period
  • Caregiver training: Guidance and tips on the needs of a palliative patient, and how to care for them at home
  • Advance Care Planning (ACP)
  • 24/7 clinical services
  • Equipment loan
  • Bereavement care

 

If you need some support for short-term intensive care or short break from caregiving, Home-Based Respite Care (HBRC) may help.
